Your OA/TOF Passport

Created by Dr Caroline Love, Chair of TOFS Medical Liaison & Research sub-committee with support from volunteer members: Amreeta Aytain, Joanna Woodley, Joanne Winspear and Tamasine Swan.

The OA/TOF Passport can be used by everyone impacted by OA/TOF.

It could be especially useful if your teen is approaching transition to adult care and will need a summary of his/her care whilst in paediatric services.

It has been designed so it may be completed by adults born with OA/TOF and the parents/carers of children born with the condition.

We hope it will support people of all ages within health and social care settings or in education. It will help you keep an ongoing record of the impact that being born with OA/TOF born has had on you or your child’s life and to assist in communicating this to medical professionals or others who may be unfamiliar with your condition. 

You can complete the Passport on your own, with support or someone else could complete it for you.

We’ve tried to keep the Passport as a comprehensive but easy to complete document that can be printed out or presented digitally.

A simpler solution is available in our Pocket-sized health record from the TOFS shop but we encourage you to use this more comprehensive record. 

What are the benefits in having an OA/TOF Passport?

The OA/TOF Passport can:

  • Help support you or your child in managing healthcare needs
  • Share vital information about your condition
  • Help to communicate specific health and personal needs to medical professionals.

How to use

Your OA/TOF Passport may be completed by typing in online or printing out and completing by hand. You do not have to complete every section if you do not want to. Just add the information you want people to know about you or your child. You can download each section individually if you prefer.

The OA/TOF Passport is unique to you. Please use it however you feel is best.

Important:

If you choose to complete the form online, please remember to save a copy of the completed form to your device (or print it out). No data will be retained by TOFS and any unsaved changes will be lost.

If you prefer to complete the document by hand, simply download and print off a copy.
